How to Become an Authorized Driver

Risk Management is responsible for the screening and authorizing of each Potential Driver for USI. A Potential Driver means any new or established employees, students, student workers, or volunteers who drive for USI as part of their job responsibilities or requested duties. Based on the criteria in the Vehicle Safety Policy (VSP) and the provisions of the University Handbook, all employees, students, or volunteers who drive on USI business, must be an Authorized Driver. 

Supervisor Action

The supervisor should make all Potential Drivers aware of the USI’s Vehicle Safety Policy. Supervisors must provide the following Potential Driver information to Risk Management:

  • Potential Driver Name, Banner ID#, and Potential Driver Email Address
  • Specify the Vehicle and Safety Training Modules the Potential Driver must complete for job responsibilities or requested duties:
    • Defensive Driving is MANDATORY for USI Drivers
    • Golf Cart and Utility Vehicle Safety
    • 15 Passenger Van Safety
  • Action - Complete the Supervisor PD Notification Form here

Additional Information for Supervisors:

  • Once Risk Management receives the Supervisors PD Notification form, an email will be sent to Potential Driver with “Potential Driver Instructions” and access codes.
  • Supervisors/Departments are to report to Risk Management, if they become aware that an individual had an accident while driving on University Business, had their driver's license suspended, or if any other event occurs that could affect their driving privileges. Authorized Driver Status

The Supervisor and Potential Driver will be notified by Risk Management once all necessary steps are completed and Motor Vehicle Reports (MVR) are reviewed.  

Driver License

When an Authorized Driver renews their Driver License, the Authorized Driver should email a copy showing the new expiration date to USI1riskmgt@usi.edu  or they may send a hard copy in Campus Mail to Risk Management. IMPORTANT: Risk Management does not keep a record of Authorized Driver Licenses on file.  All copies of Renewal Driver License are destroyed.

Self-Disclosure Form

All Authorized Drivers must disclose a driving offense that could potentially affect the individuals’ ability to perform the duties, safely or effectively, in positions that drive a university vehicle in any capacity. Any driving offense, arrest, or conviction must be submitted within seventy‐two (72) hours. The Disclosure Categories include traffic citations/moving violations, suspension/loss of driver’s license, and driving while under the influence. Major Offenses include traffic accidents with bodily injury or fatality, traffic accidents involving alcohol/drugs, or other similar charges/convictions.

  • Supervisors/Departments are to report to Risk Management if they become aware that an individual on their driver's list has had an accident while driving on University Business, had their driver's license suspended, or if any other event occurs that could affect their driving privileges.
