Graduation Information

In order to make sure that you are on track for graduation, you need to do the following:

• Make an appointment with your Health Services Advisor to complete the Formal Application for Graduation Form.

• Complete the Online Diploma Form (requires myUSI username and password). Make sure you select whether you want USI to mail your diploma to you or if you prefer to pick it up in person.

The Diploma Form contains your graduation information, including your name as you want it to appear on your diploma and in the commencement program (proper spelling, upper/lower case, middle name or initial, etc).

If no Diploma Form is submitted, the student's current name on record with the University will be used when printing the commencement program and diploma.

Deadlines

If degree requirements will be completed in a spring or summer term, submit forms by October 1 of the preceding fall term.

If degree requirements will be completed in the fall term, submit forms by March 1 of the preceding spring term.
