Case Management/Care Coordination Program

Description of program
Case management strives to assist clients and families to navigate through healthcare systems. Case managers provide advocacy for clients and interact with the members of the interprofessional healthcare team to find solutions in providing effective, quality, and efficient care.  Case managers provide care coordination to communicate the needs of the clients and guide the delivery of appropriate and effective care.

The program was designed utilizing the Standards of Practice for Case Management as endorsed by the Case Management Society of America.

Who should enroll in this program?
This program is designed for English-speaking nurses and other healthcare professionals who want to enhance or develop the knowledge and skills necessary for quality case management.

Program Content
The content is divided into modules. Each module utilizes audio/video recordings, reading assignments, articles, web resources, and case studies to augment the learning process. These methods provide the participant with flexibility in completing assignments and interacting with the instructor.  Each module has an accompanying study guide with questions (such as multiple choice and essay) for participants to submit to the instructor.  Interaction with instructor may occur via study guides, discussion boards, and or email. The modules cover case management principles, case management process, care coordination of services, financial aspects of case management, disease management, and application of care management.

A textbook is required for this program. Upon registration, you will receive detailed information concerning the textbook.

Course Schedule
This eight-week activity is offered twice a year.  The deadline for registration is the Wednesday prior to the start date.

Learning Outcome
The learning outcome of this program is to enable the participant to successfully implement care management (care coordination) practices, ethics, and standards in their clinical settings through interprofessional collaborative practice.

Program objectives
At the conclusion of this program, the participant will be able to: 

  1. Present an overview of the case management process.
  2. Identify methods to coordinate services to meet client needs.
  3. Explain the financial aspects of case management.
  4. Integrate disease management and case management into the plan of care.
  5. Apply case management principles to clinical situations.
  6. Assimilate evidence based interprofessional collaborative practice.

Delivery of content
The content is delivered asynchronous via the online learning management system, Blackboard. The internet provides an opportunity for interaction with program faculty while in the comfort of learning in your own home and office.  Program staff are available during business hours by phone or email to assist participants with technical issues or concerns. 

Note: Many businesses, especially government agencies or health institutions, utilize security firewalls.  Access to web links within program and communication from the instructor, or other students may be blocked. We recommend not using your work email address for this course.

Contact hours

The University of Southern Indiana Center for Health Professions Lifelong Learning designates this activity for 40 contact hours.   

Criteria for successful completion

  • Submit registration and payment.
  • A certificate for contact hours will be provided following completion of each module study guide, receive a passing score of 70% or better on comprehensive final, and submission of program evaluation.

The certificate of completion will be sent via email immediately upon the completion of the program evaluation.

Successful completion of this program does not guarantee expertise in the content. Clinical experience and judgment coupled with an understanding of the content are necessary to attain competency. Achieving certification via a designated professional organization is an additional step to demonstrate competency.

This program has been pre-approved by The Commission for Case Manager Certification to provide continuing education credit to CCM® board certified case managers. This course is approved for 40 CE Contact hours.
