In the event of a utility failure, immediately notify USI Facility Planning and Operations (812-464-1729 or ext. 1729) and USI Public Safety (812-492-7777 or ext. 7777). You should be prepared to provide the following information:
All buildings are equipped with a temporary emergency lighting system that provides enough illumination in corridors and stairs for safe exiting. Evacuate the building while the emergency lights are on. If you are in a laboratory equipped with a fume hood and/or biosafety cabinet, close all hoods and biosafety cabinet sashes. Do not enter the building until power is restored. It is advisable for departments to have flashlights.
If you become trapped in an elevator, use the emergency phone to notify Public Safety. Turn on the emergency alarm, which will signal your need for assistance.
If you discover leaking water or know the source of a water leak, contact Facility Planning and Public Safety. Cease using electrical equipment. Do not use the elevator. Avoid the area and do not walk through standing water because of potential for electrocution.
If you discover a natural gas leak, cease all operations. Do not switch on lights or any electrical equipment. Evacuate the area immediately and notify Facility Planning and Public Safety. Do not use the elevator.
If necessary, cease operations and evacuate the area. Immediately notify the Facility Planning and Public Safety.
If smoke or odors come from the ventilation system, immediately notify the Facility Planning and Public Safety. If necessary, cease all operations and evacuate the area. Follow General Evacuation Procedures.
In the event of a technology failure (network, computer or telephone), contact USI Information Technology (812-465-1080) or the IT Help Desk and request immediate escalation to IT senior management. Additionally, USI Public Safety (812-492-7777 or ext. 7777) may be notified. If the event is information security related, refer to the Information Security Incident Response process outlined in the IT Information Security Policy.
